Professional Consulting Services

Initial consultations consist of a one-hour consulting session for individuals or families to provide counseling and direction. Licensed and Certified Geriatric Care Managers are available to review all social, financial, medical, and legal issues and create a customized plan for you. Our extensive knowledge base can provide you with a referral network of providers, physicians, specialists, psychiatrists, and Independent Living, Assisted Living, or Nursing Home communities. If an independent or assisted living community or nursing home is chosen, we can assist you with the paperwork and the transition to placement. Our professionals will give you the tools and resources to optimize your benefits. We can also negotiate with communities to reduce your community fee and/or rate. Families who come to us always say the same thing – wishing they had come to us sooner because they were headed in the wrong direction and could have saved so much time.

Home Care Management

Our team manages medical appointments, prescriptions, transportation, and in-home care. A Licensed and Certified Geriatric Care Manager will create and develop a team of professionals to keep you safe and in your home for as long as possible. Our home team professionals include Registered Nurses, Licensed Professional Nurses, Client Service Specialists, Meal Coordinators, and Care Managers. We create a system that is efficient and organized. We can keep families connected, providing information on a routine basis.

Probate Court Practices

Our licensed and certified staff serve as professional guardians. We also provide expert testimony. Our team of experts has provided services for the Upstate for the past 30 years.

Assessments

A Licensed and Certified Geriatric Care Manager and Registered Nurse will complete a comprehensive assessment and provide recommendations for continued care. This comprehensive report covers legal, social, financial, dementia & cognitive testing, insurance, and medical issues. We are a mobile geriatric unit. Whether you are in a home, a facility, or a hospital, we come to you. For families, it can be challenging to know what to do, especially in a crisis. Let our professionals guide you in the right decision. Home safety assessments and small changes can help you stay independent longer. Facility Care Management

Our team attends care plan meetings, makes on-site visits, submits reports to family members, and manages overall care.

Special Needs Trust (SNT)

Our team provides Case Management, Registered Nurses, and medical oversight of Special Needs Trusts. We are a patient advocate to the recipient, assisting with managing prescription medications, adaptive equipment needs, transportation, home visits, and medical appointments. Our professionals also provide ongoing advisement to the Trustee group managing the SNT. Special Needs Trust services are available for clients of all ages, from pediatric recipients to young adults and beyond.
